The Tufts Health Plan Foundation’s Healthy Aging Grants Program seeks to fund programs that help older adults live healthier lives. The Program is divided into three categories: Health & Wellness, Purposeful Engagement, and Empowerment. The Health & Wellness category focuses on promoting chronic disease self-management and prevention, exercise and nutrition, and mobility, strength and balance for older adults.

Since the foundation began focusing its grantmaking on healthy aging, it has given nearly $12 million to nonprofits that elevate the physical, mental and emotional well-being of older adults and their caregivers. Cycle 1, 2013 grants included $1.6 million in new funding to 34 Massachusetts and Rhode Island-based organizations.
